EnhanceWellness

EnhanceWellness is an individualized health coaching program for older adults with multiple chronic conditions. A one or two-person team (usually a social worker and a registered nurse) work with the participant to motivate behavior change over a six-month period.  

The program complements formal healthcare services, and is designed to work seamlessly with other evidence-based programs. EnhanceWellness is made up of three parts:

• Screening: The EnhanceWellness staff member and the participant meet and review a detailed health questionnaire.
• Planning: The participant, with staff coaching, creates a plan that focuses on areas he or she wants to work on. The plan is shared with the participant's doctor.
• Action: As the participant follows the plan, a health professional and/or volunteer health mentor offers encouragement, feedback, and monitoring. The staff help with problem solving, health education, and referral to support groups and additional services, such as individual and family counseling, when needed.
